How Do I Fill Out a Product Approval Information Sheet, or PAIS?

A product approval sheet requests the relevant information and product approval numbers for building components listed that pertain to your construction project. Vendors used during the construction process will provide product information.

If vendors do not provide the information, read ahead to the “How can I find my product information?” section to learn how to search within Florida’s database.  

 

Only if there is exterior work to be done, will you need a product approval sheet. For example, if there are windows, soffits, or a new door, a product approval sheet must be filled out and submitted. If there are any building components that have outside exposure, each item you have must be listed on the sheet in complete accuracy.  

All of the columns including manufacturer, product description, code version, FL# (with suffix), and expiration must be filled out. If the columns are not filled out to the full extent, COJ is within their right to deny the submission and delay permit issuance.

The first step to gaining information to fill out your sheet is to contact your vendor to obtain the FL#. To gain further information of the products, click the link to visit the Florida Department of Business & Professional Regulation’s database on the approved product’s site. 

The database allows you to search for FL#s and other information about your products. 

If you do know the FL# of your product, enter it in the highlighted area and further information will appear. Product information is accessible by looking up the FL# and the results will be used to completely fill out the product approval sheet.
